Questions to Ask When…..

Looking For A Geriatric Care Manger

If the problems you are facing with a senior loved one become larger and more complex than you can manage, or if you are unable to spend the time and attention necessary to find the solutions you need, a geriatric case manager may be a solution.

WHAT DO GERIATRIC CASE MANAGER DO?

  • They can conduct care-planning assessments to identify problems and to provide solutions.
  • Screen, arrange, and monitor in-home help or other services, including assistance in hiring a qualified caregiver for home care.
  • Provide short- or long-term eldercare assistance for those engaged in local or long distance caregiving.
  • Review financial, legal, or medical issues and offer referrals to those specialists.
  • Provide crisis intervention.
  • Act as a liaison to families at a distance, overseeing care, and quickly alerting families to problems – especially important when families are engaged in care for a loved one and live at a distance.
  • Assist with moving an older person that requires a change in levels of care.
  • Provide consumer education and advocacy.
  • Offer counseling and support.

 

Some Geriatric case managers also provide family or individual therapy, finance management, guardianship assistance, and/or caregiving services.

Here are questions to ask when looking for a Geriatric Case Manager:

  1. What are your professional credentials?
  2. Are you licensed, or supervised by someone?
  3. How long have you provided care management?
  4. Can you be reached in case of emergencies?
  5. How will we communicate?
  6. Will I sign a written contract? What are your fees?
  7. Do you have references?
  8. Can you give me a written care plan?
  9. Do you charge for conducting an assessment?
  10. What is your knowledge of community programs and services?
